In this study, trepanations in ancient Anatolia were discussed from a historical perspective. Trepanations were studied in respect to temporal and spatial distribution, sex and age distribution, techniques and reasons, completeness, healing and number of holes. Forty individuals from 23 different Anatolian settlements are identified to have undergone trepanations. Cranial trepanations in Anatolia show a distribution over a period of 10 000 years ranging from the Aceramic Neolithic period to the Late Ottoman period and spread to whole Anatolia. The greater majority of the individuals had single trepanation orifices while only four individuals were identified with multiple holes. It is observed that the surgical procedure was predominantly carried out on males in Anatolia. Main techniques of trepanations used in Anatolia are drilling and cutting. Early cases of trepanation were made by drilling; however, this technique has been used for cranial surgery until the Ottoman period. Scraping and rectangular sawing techniques first applied in the Early Bronze Age. The boring‐and‐cutting technique was only applied in the Iron Age. More than half of the trepanations practiced due to cranial trauma. Training and treatment besides of cranial trauma are also considered as likely causes of trepanations in Anatolia. We present an outstanding example of successful prehistoric double trephination dating between 2700 and 2200 BC, most likely to the Corded Ware culture, at the end of the Neolithic Age. The particularity of this case is the presence of a double trephination, one frontal over the sinus sagittal superior and one parietal right. There is evidence that the patient survived months to years after the operations. The purpose of the procedure is not known. The case confirms the astonishing degree of technical skills reached in Saxony-Anhalt over 4500 years ago without anesthetic, antiseptic, or technologic aids.  相似文献

In 1894 at St. Vincent's Hospital Melbourne, George Adlington Syme removed a meningioma from a patient with symptomatic focal epilepsy. The operation stands as the first surgery based on seizure localization in Australia. It is also the country's first documented successful resection of an intracranial meningioma. It followed William Macewen's landmark cerebral localization case on the boy John McKinley by 18 years and Victor Horsley's first epilepsy case on Hughlings Jackson's patient James B. by a mere 8 years. Syme's patient, Constable John G., survived the operation by some 23 years, dying from a gunshot wound to the head in 1917. Newly discovered inquest papers reveal that the coroner's judgment that the death was accidental completely fails to address the more credible scenario of suicide. The story makes for a fascinating epilogue to an important landmark in Australia's neurosurgical history.  相似文献

This article highlights the major reflections of ancient Greek mythology in modern neuroscience. An analysis of ancient Greek texts and medical literature using the MeSH term mythology was performed to identify mythological references pertaining to neuroscience. The findings are discussed in relation to etymology, early conceptualization of the nervous system structure and function, incipient characterization of neuropsychiatric disease, and philosophical stance to the practice of medicine in ancient Greece. The search identified numerous observations in clinical neurology (e.g., stroke, epilepsy, cognitive and movement disorders, sleep, pain and neuromuscular medicine, neuroinfectious diseases, headache, neuroophthalmology, and neurourology), neurosurgery, and psychiatry, as well as basic neurosciences (e.g., anatomy, embryology, genetics, pathology, and pharmacology) concealed in ancient myths. Beyond mere etymological allure and imaginative reflections in science, these fables envisage philosophical concepts that still tantalize our protean medical practice today.  相似文献

ABSTRACT

Trephines and trepanning date to ancient times, but a “modern” form of instruments was codified by the seventeenth century. This did not preclude efforts to “improve” the trephine in the late-eighteenth and nineteenth centuries. Surgeons and instrument makers in Britain (Jardine and Savigny), France (Thomson and Charrière), and America (Galt and Otto & Reynders) endeavored to make the trephine safer and more precise. In exploring their interactions, this presentation shows the evolving role of the instrument makers not only as fabricators of tools, but as creative design collaborators of surgeons and physicians.  相似文献
