The aim of this paper is to suggest an analytical framework through which the complex question of hospital closure and local protest can be further explored. The significance of this area of study lies with the extraordinary events that surrounded a campaign to save the Kidderminster General Hospital from downsizing. Whilst the hospital was not saved, the local campaign committee organizing the protest took their fight into the political arena and were successful in gaining a parliamentary voice in the 2001 general elections. Such an achievement has had considerable political implications, however, the question asked here is how might we begin to interpret the events that took place. To this end, the paper draws on current geographical literature in an attempt to construct a framework that accounts for both national policy debates and the local context within which such campaigns emerge. 相似文献

新时期农村合作医疗经历了曲折的发展历程。随着家庭承包经营的推行,农村合作医疗迅速瓦解,覆盖率由20世纪70年代末的90%下降到1985年的5%。90年代初开始,党和政府努力恢复合作医疗,但重建工作一再受挫。农民自费医疗的境况带来了严重的社会问题。2003年初,新型农村合作医疗制度出台,这是又一项富有中国特色的社会主义制度创新。几年来的试点和推广实践证明,新型农村合作医疗制度符合我国的实际,对保障农民身体健康、维护社会和谐和促进经济发展具有重要意义。同时,建立完善的农村医疗保障制度需要各方力量的整合,任重而道远。 相似文献

When studied through canon law and scholastic pastoralia produced in the universities in the thirteenth century and beyond, medieval pastoral care comes across as spiritual care, more specifically the administration of sacraments and preaching, provided by the clergy for the faithful. This article complicates that view by arguing that in the twelfth century, the laity alongside the clergy was active in the provision and organisation of pastoral care. The sources examined are the surviving statutes of five religious confraternities – along with the obituaries and sermons in two cases – in Italy that flourished in the twelfth century and before. Each of these confraternities was centred around a church, established after an apostolic ideal, included laymen and women and local pastoral clergy of all levels, met regularly to celebrate the Eucharist, prayed for the dead members and made public confessions. Members prayed for and attended to the corporal needs of each other in case of sickness. In the final analysis, these twelfth-century confraternities appear as transitional institutions between the early medieval monastic confraternities focusing on prayer and the late medieval and renaissance confraternities focusing on charity. Their study opens a window onto the lay expectations of and contribution to pastoral care in medieval Italy. 相似文献

This article explores imagined geographies of health care among Lesbian, gay, bisexual, transgender and queer (LGBTQ) women in Halifax and Vancouver. In expressing the possibilities and limitations of accessing care, participants frame their own experiences through spatialized narratives of how LGBTQ people are thought to be treated elsewhere. Participants’ explicit connections and distinctions between Halifax and Vancouver gave insight into how their perceived difficulties in one health care context are sometimes framed by imagining more ease in accessing care in the other. We explore here the implications of these imagined, idealized spaces, which may set up false expectations that things are always better elsewhere. We reveal imagined geographies and senses of place as highly relevant features in LGBTQ women’s accounts of experiences with and access to health care and expand conventional arguments about physical access to care. 相似文献

This article compares the interrelationships between gender, family structures and intra-family care arrangements during two markedly different periods of Albania's recent history. The first of these, the communist era, was dominated by the autocratic state-socialist regime of Enver Hoxha. In contrast, the post-communist period that followed was characterised by a kind of reactive free-for-all capitalism and high rates of both internal and international migration, the latter mostly to Greece and Italy. Families have been torn apart by this mass emigration, resulting in husbands separated from their wives and children, and older generations left behind by their migrant children. All this contrasts with family, residential and care arrangements during the communist period when not only were families generally living in close proximity, but also state welfare was available to support vulnerable and isolated individuals. Across these periods, however, the burden of care responsibilities fell almost wholly on women, despite the egalitarian ideology of the socialist era and the potentially modernising and empowering effects of post-socialist migration on the agency of women. The article provides a valuable lesson in historicising regimes of gender, family and care across dramatically contrasting social models. 相似文献

This article makes a case for a ‘buddy system’ approach to research and scholarship, or a kind of ‘caring with’ our colleagues, as feminist praxis and as an intentional, politicized response to the neoliberalization of the academy. Through autoethnographic writing on our travels together into farmed animal auction yards, we explain the buddy system as a mode of caring, solidarity, and love that differs from collaborative research, focused as it is on caring for and about our colleagues and their research even (or especially) when we have no direct stakes in the research being conducted. We contribute to three feminist conversations with this approach: feminist care ethics in geography; emotional geographies; and critical perspectives on the neoliberalization of the academy. We advocate the buddy system as an extension of feminist care ethics, enriching how feminists think about ‘doing’ research. We draw on feminist geographies of emotion and our own emotions (grief especially) experienced while witnessing processes of nonhuman animal commodification to politicize the act of researching and to develop a more caring way of inhabiting the academy. This is particularly important, we argue, in the context of deepening neoliberal logics that turn the academy into a place where care and love become radical acts of resistance and transformation. 相似文献

This article analyzes the multiscalar relationship between state violence and domestic violence among low-income residents of Cairo, Egypt. To do so, I look at the concept of care and how notions of care are intimately linked with interpretations of violence within daily life. In the accounts of research participants, acts of violence are instances that are understood as lacking regard for the other’s well-being. Violence is juxtaposed against ‘discipline’ and is generally understood as an act of care meant to correct inappropriate behavior. Here, discipline can encompass acts of physical force. Finally, findings show that for low-income Egyptians, the violence of the state not only shapes but also happens simultaneously to violence in the home. 相似文献

This study deployed statistical and spatial (GIS) analyses to test the effects of geographic, demographic and occupational factors in predicting health centre visits among the elderly (≥60 years) with chronic ailments in Kanchanaburi Province, western Thailand. Datasets from the Kanchanaburi Demographic Surveillance System (2004 baseline survey and a 2006 project follow-up) comprised five land-use landscape and occupational strata: urban/semi-urban (industrial), rice field, plantation crop, upland and mixed economy. Travel distance to the nearest health centre was calculated following transportation routes and the GPS coordinates of individual home locations, and the number of healthcare visits under the national free universal coverage (UC) scheme reported for the previous year was used as a dependent variable in regressions. Broadly, the study found that distance was not a significant predictor of the rate of elderly health centres visits for those with co-residing spouses and/or adult children (77% of sample), and for the elderly still working (48% of the sample) who were the least likely to access healthcare. Owning motorised vehicles did not encourage visits, which also varied by land-use and occupational strata. 相似文献

This study applies the concept of care to examine how home–work transitions of high-tech men affect others in these two places, namely their wives and managers. The high-tech industry is famous for its particularly demanding culture and masculine disposition, which contest daily involvement with family and domestic affairs. Care is conceptualized as a wide-ranging multifaceted notion that embraces work, morals, and policy, and is represented by the exchange of various tangible and intangible, resources across the home–work divide. In-depth interviews with 22 high-tech managers and 47 wives of high-tech engineers disclose a well-established reciprocity of care resources. The managers reward the wives' nonmaterial support of the engineers/husbands with rhetorical recognition and nonfinancial benefits. The spatialization of care across the home–work divide is discussed, pointing to its hierarchical – not only contextual – relations. 相似文献

Like all spaces, concrete caring places both shape and are shaped by understandings and constructions of normativity and identity. The traditional understanding of care for older people, imagining clearly demarcated dyadic roles, is firmly embedded in heterosexual logics of relationships within families, the own (family) home and institutional support. Social and residential places for older people thus both assume particular gender and sexual identities and contribute to a (re)production of the very normativity. But how can this interlinkage between the construction of caring spaces and the normativity of identities be understood and, possibly, challenged? In this article we discuss the transformative potential of the social (and partly residential) space of La Fundación 26 de Diciembre, in Madrid, Spain, which opened up to specifically support older LGBT people. Drawing on an in-depth case study we explore a space that allows visibility of different forms of living and caring practices of people with different genders, sexual preferences, origins, classes or political backgrounds. Through the daily life narratives of the people who work, volunteer or simply use the centre we discuss the potential of challenging the restricted notions, assumptions and constructions through which particular places gain both social and political meaning. The article highlights the transformative power of the active and collective making of caring spaces through which narratives of care, collective sexual and gender recognition and practices of caring relationships can replace both traditional/informal forms of living together and institutional spaces that provide professional care. 相似文献
