Localisation and William Macewen's early brain surgery Part I: the controversy

Neurosurgery for the removal of brain tumours based on localising signs is usually dated from the 1884 operation by Bennett and Godlee. However, within weeks of that operation claims were made on behalf of William Macewen, the Glasgow surgeon, to have been the real pioneer of such surgery. According to Macewen's protagonists, he had conducted seven similar operations earlier than Bennett and Godlee and, in a notable 1888 address, Macewen described these seven pre-1884 cases and a number of others operated on after 1884. This paper, which is in two parts, contains an evaluation of the claims made for the priority of Macewen's pre-1884 operations. Part I deals mainly with Macewen's work in fields other than brain surgery that are relevant to it and sets out the facts of the controversy. It begins with a brief biography of Macewen, describes his pioneering work in antiseptic and aseptic surgery, his work on osteotomy and bone regeneration, and his use in brain surgery of the knowledge so gained. Part I concludes with an examination of the battle waged in the newspapers between Macewen's and Bennett's and Godlee's supporters, and of previously unpublished correspondence between Macewen himself, David Ferrier and Hughes Bennett. The primary records of the patients on whom Macewen operated, together with other materials relevant to the controversy, are examined in Part II.     

Metamorphic and basin fluids in quartz–carbonate–sulphide veins in the SW Scottish Highlands: a stable isotope and fluid inclusion study

Metalliferous (Fe–Cu–Pb–Zn) quartz–carbonate–sulphide veins cut greenschist to epidote–amphibolite facies metamorphic rocks of the Dalradian, SW Scottish Highlands, with NE–SW to NW–SE trends, approximately parallel or perpendicular to regional structures. Early quartz was followed by pyrite, chalcopyrite, sphalerite, galena, barite, late dolomite–ankerite and clays. Both quartz–sulphide and carbonate vein mineralisation is associated with brecciation, indicating rapid release of fluid overpressure and hydraulic fracturing. Two distinct mineralising fluids were identified from fluid inclusion and stable isotope studies. High temperature (>350°C) quartz‐precipitating fluids were moderately saline (4.0–12.7 wt.% NaCl equivalent) with low (approximately 0.05). Quartz δ¹⁸O (+11.7 to +16.5‰) and sulphide δ³⁴S (?13.6 to ?1.1‰) indicate isotopic equilibrium with host metasediments (rock buffering) and a local metasedimentary source of sulphur. Later, low‐temperature (T_H = 120–200°C) fluids, probably associated with secondary carbonate, barite and clay formation, were also moderately saline (3.8–9.1 wt.% NaCl equivalent), but were strongly enriched in ¹⁸O relative to host Dalradian lithologies, as indicated by secondary dolomite–ankerite (δ¹⁸O = +17.0 to +29.0‰, δ¹³C = ?1.0 to ?3.0‰). Compositions of carbonate–forming fluids were externally buffered. The veins record the fluid–rock interaction history of metamorphic host rocks during cooling, uplift and later extension. Early vein quartz precipitated under retrograde greenschist facies conditions from fluids probably derived by syn‐metamorphic dehydration of deeper, higher‐grade rocks during uplift and cooling of the Caledonian metamorphic complex. Veins are similar to those of mesothermal veins in younger Phanerozoic metamorphic belts, but are rare in the Scottish Dalradian. Early quartz veins were reactivated by deep penetration of low‐temperature basin fluids that precipitated carbonate and clays in veins and adjacent Dalradian metasediments throughout the SW Highlands, probably in the Permo‐Carboniferous. This event is consistent with paragenetically ambiguous barite with δ³⁴S characteristic of late Palaeozoic basinal brines.

Paracondylar process: Manifestation of an occipital vertebra

In the examination of over 1300 burials from medieval Canterbury, one individual displayed a well-defined paracondylar process. Four other adults and two children presented with small tubercles at the same site. The aetiology of the trait is discussed with reference to cranial base variants and the so-called manifestation of an occipital vertebra.     

Localization and William Macewen's early brain surgery Part II: The cases

This paper contains Part II of an evaluation of the claims made for the priority of Macewen's pre-1884 brain surgery over that of Bennett and Godlee. Although the primary sources--Macewen's Private Journals and the Ward Records of the Glasgow Royal Infirmary--confirm that the operations were carried out when Macewen said they were, problems with the sources make it difficult to evaluate the extent to which he actually used knowledge of localization bearing on the claim for his priority evaluated. Part II concludes with citations from archival correspondence and the contemporary and near contemporary medical press confirming Macewen's priority.     

The Liberalization of the Land Market in Spain: The 1998 Reform of Urban Planning Legislation

Against a general climate of liberalization, the Spanish Government has made a concerted effort to bring about a reduction in house prices. A legislative reform in April 1998 has sought to liberalize land and planning, through (a) refining the category of land previously excluded from development to enable residential development to take place, (b) allowing for greater flexibility of land uses and building controls, and (c) reducing administrative controls. This paper discusses the extent to which such reforms are likely to meet their anticipated objectives, resulting in beneficial effects in the land and housing markets, as well as the territorial impact of such reforms from a sustainability perspective.
