The prevalence of osteoarthritis, osteophytosis, intervertebral disc disease and diffuse idiopathic skeletal hyperostosis (DISH) was determined in a group of skeletons excavated from the crypt of Christ Church, Spitalfields. Age-specific prevalence rates were calculated from the group of skeletons for which age at death was known from intact coffin plates. Osteoarthritis of the facet joints occurred most frequently in the cervical and upper thoracic regions and was also common in the lower lumbar spine. The disease was more frequently bilateral in the lumbar spine than in upper regions. Osteophytes were common in the thoracic and lumbar spines, whereas intervertebral disc disease was confined almost completely to the cervical region. Diffuse idiopathic skeletal hyperostosis seemed to be underrepresented in this group, but was more common in males than in females. Age correlated significantly with all the conditions except DISH. Multiple regression analysis was carried out using each of the conditions as the dependent variable in turn. Age accounted for more of the variance than any of the other conditions added as independent variables.  相似文献

The distribution of osteoarthritis of the hands was studied in 101 skeletons from archaeological sites in England. The results are similar to those found in epidemiological studies of modern populations. Osteoarthritis of the hands was more common in females than in males, and in females tended to involve a greater number of joints. The distal and proximal interphalangeal joints were commonly affected, the former more frequently than the latter, but otherwise the disease was largely confined to the first ray of the hand, centred on the trapezium. It is concluded that the pattern of osteoarthritis in the hand has probably not changed greatly over several hundred years.  相似文献

Osteoarthritis is a major health concern in living populations, as well as being one of the most common pathological lesions identified in the archaeological record. The aetiology of the disease remains unclear, with a multi‐factorial influence of physical strain, age, genetics, and obesity. Previous studies have identified a relationship between the presence of knee osteoarthritis on the distal femoral joint and the morphology of the intercondylar notch, patellar groove, and medial condyle. The current study expands this research to investigate the relationship between distal femoral, distal humeral, and proximal ulnar joint morphology and osteoarthritis with 3D shape analysis techniques. These methods provide a more detailed analysis of joint morphology in order to determine any relationship between 3D shape and osteoarthritis. The results indicate a complex relationship between joint shape and knee osteoarthritis, with eburnated right femora showing a statistically significant association. The shapes associated with eburnated or affected femoral joints can be explained by osteophyte development, and therefore likely represent systematic shape changes and not a particular joint shape predisposing individuals to the condition. There was no identifiable relationship found in the proximal ulna or distal humerus, indicating that joint shape is unlikely to influence the development of the condition in the elbow joint and that any shape changes produced by osteoarthritis are not systematic or quantifiable. The joints analysed in this study were highly influenced by asymmetry, sexual dimorphism, and allometry, resulting in a small sample size of affected joints in many datasets. Further analyses of large skeletal samples are needed to more thoroughly investigate the possible relationship of distal femoral joint shape and osteoarthritis. The prevalence and distribution pattern of Schmorl's nodes (SNs) were studied in a post‐medieval skeletal sample (n = 473) from the 16th–18th century cemetery of Klostermarienberg, Austria. The reasons for the prevalence and distribution pattern of SNs in this sample are discussed with regard to their aetiology. SNs were correlated with age and sex as well as with degenerative spinal joint disease such as vertebral osteophytosis (VO) and apophyseal osteoarthritis (OA). SNs were most commonly found in the lower thoracic region, in agreement with other studies. Males were more affected than females by SNs, especially in the lower thoracic region. SNs show a completely different distribution pattern to VO and OA. Additionally, there was no relationship found between SNs and ageing. Observed differences in the prevalence of SNs in the vertebral column and between the sexes suggest that mechanical factors may be responsible. Copyright © 2008 John Wiley & Sons, Ltd.  相似文献

The antiquity of rheumatoid arthritis (RA) is still in dispute, due to the difficulty of conclusively differentiating peripheral polyarthritis, especially from spondyloarthropathy, in archaeological populations. In view of the importance of genetic factors in developing spondyloarthropathy and of the far lower prevalence of the disease in modern Japanese, a rarity of peripheral polyarthritis would be anticipated in the ancient Japanese population, given that RA had not been present there. One hundred and sixty adult Japanese skeletons of the late and final Jomon period (3400–2400 years bp ) were examined to find peripheral polyarthritis. There was one male skeleton with peripheral polyarthritis, showing marginal and surface erosions in the joints of the hands, feet, ankle and zygoapopheseal joints of the thoracic vertebrae. RA is suggested as the most probable cause of this pathology. This report raises the significance of further studies to consider whether more cases than anticipated exist in skeletal populations that would have been resistant to developing spondyloarthropathy. Thirty-eight participants at the VIIIth European Meeting of the Paleopathology Association took part in a study of inter-observer variation in scoring osteoarthritis in human skeletal remains. Ten specimens representing different joints were used and five criteria of osteoarthritis were scored. Eleven of the 38 participants ranked themselves as beginners, 13 as experienced and six as very experienced; the data were subsequently examined using the results from these 30, comparing beginners with experts. Agreement as to whether or not changes were present on the specimens and on the degree of change was seldom complete but was greater when scoring eburnation and the presence of new bone on the joint surface than for the three other criteria. There was little difference between beginners and experts. Although all the specimens were chosen to meet our published criteria for osteoarthritis, the experts were unanimous is agreeing the diagnosis in only three cases and the beginners in only one. These results suggest that more work needs to be done to develop operational definitions for the classification of disease in palaeopathology and that great care must be taken when comparing disease frequencies between studies.  相似文献

Apparently-articular facets on the undersurface of the anterior third of the acromion have been known in skeletal material since 1922 but without full discussion of the mechanism responsible. Twelve instances are described here, selected because an impingement area could be demonstrated on the greater tubercle of the humerus or on osteophytes that had replaced it. Both sides were affected but, with two exceptions, the right more severely; the less affected side could be regarded as an earlier stage. All were affected by osteoarthrosis. One was complicated by coraco-humeral impingement. The anatomical mechanism that normally prevents the humerus from rising against the coraco-acromial arch is described as well as the causes of the breakdown in the mechanism that leads to the impingement, derived largely from the clinical field, where the disorder is well-known. The complexity of the relation of the disorder to degenerative arthrosis is discussed because the degeneration is an age change in which wear and tear plays a localized part and the changes as they affect the shoulder joint differ fundamentally from those in the weight-bearing hip-joint. The mean estimated age of the group of twelve was 59 years but the occurrence of the acromion impingement disorder in young athletes illustrates the part played by mechanical joint abuse. Women predominated in this small group, partly because, through their longer life-spans, their joints were exposed longer to the susceptible period of age-related degeneration and also because their less robust musculo-skeletal systems were less adapted than men's to the equally shared labour. The designation of the Klebs-Loeffler bacillus as the cause of diphtheria in the early 1890s and the subsequent development of the antitoxin treatment in the years immediately following were at the time and continue to be viewed as triumphs of scientific medicine. I focus on these two developments to illustrate the problems that arise in attempting to answer the questions regarding the role that changes in medical practice--in this case, the use of antitoxin as a cure--played in lowering death rates at the time. Changes in diagnostic techniques, the selection of cases to be included, and ultimately the agendas of the persons constructing them affected the numerators and denominators of these rates. The data suggest that the antitoxin had some effect on already declining diphtheria death rates, but because of changes in understandings of the disease and contemporaries' presentation of the data, the size of that effect and its role in mortality decline more generally elude us. Our analysis of the past depends on numbers that reflect not only changing treatments but also changing understandings of disease at the end of the nineteenth century.  相似文献

ABSTRACT

In order to study the influence of looseness on seismic performance of through-tenon joints, six through-tenon joints specimens in 1:3.2 scale, including one tight-fitted joint and five loose joints, were fabricated according to the Engineering Fabrication Methods of Qing dynasty. The failure modes, mechanical behaviors such as the hysteresis and envelope curves, strength and stiffness degradations, energy dissipation, and deformation of the specimens under low cyclic reversed loading tests were analyzed. Results show that the failure modes of theses joints under positive and negative loadings are bending damage on the big tenon head and tearing damage on the variable cross-section, respectively. The hysteresis curves of joints appear to be anti-Z shape and present typical pinching effect. The greater the degree of looseness is, the more obvious the slippage is. The bending moment, stiffness and energy dissipation of loose joints are lower than those of the tight-fitted joint under the same rotation angle, and the increase in the loose degree cause them to gradually decrease. Meanwhile, the behavior of the through-tenon joints is further simulated by the nonlinear finite element analysis program, and the test results agree well with the numerical analysis results.  相似文献

The purpose of this research was to explore the relationship between degenerative joint disease (DJD) in the apophyseal joints and intervertebral joints in the vertebral column and assess which vertebral DJD is the more reliable indicator of general activity‐related stress. We hypothesized that the more reflecting vertebral DJD for general activity‐related stress would appear to show a higher correlation with Schmorl's nodes (SNs). To test this hypothesis, we examined 125 individuals from Eunpyeong Cemetery (mid‐15th to early 20th centuries) in Seoul, Korea and analyzed the frequencies of two types of vertebral DJD and their relationship with SNs. Cross‐tabulation chi‐square tests were used to evaluate the significance of the differences between frequencies of vertebral pathologies. In conclusion, results revealed that the relationship between the two kinds of vertebral DJD appeared differently depending on vertebral region, joint type and sex. The test results of association between the two types of vertebral DJD were statistically significant at the cervical and lumbar regions of males. SNs appeared independently of the pattern of vertebral DJD in nearly all joints with the exception of a correlation between DJD in the intervertebral joints and SNs of the lumbar region in females. As indicated in some studies, this study reconfirmed that the differences in the two kinds of vertebral DJD result from differences in normal vertebral morphology and anatomical function. This study contributes to the overall discussion on the relationship between vertebral DJD and physical activity by showing that the detailed recording and understanding of the two types of vertebral DJD is required before they can be used convincingly in a study on activity‐related stress. A simple method for calculating the prevalence of disease in a past human population is described with particular reference to osteoarthritis. It can be applied to diseases other than osteoarthritis, and should prove particularly useful when studying the epidemiology of other diseases which affect numerous sites within the body and where the overall prevalence cannot be obtained simply by adding the prevalences of the individual sites, because more than one may be affected in the same skeleton. Measures of disease frequency are discussed and their use in palaeopathology illustrated with examples, both hypothetical and real. It is shown that prevalence rates are the appropriate rates to use when calculating the frequency of diseases in skeletal populations. The extent to which prevalence rates accurately reflect those in the living population from which the skeletons were derived is also considered and it is suggested that they are a reasonable approximation for diseases that do not shorten life span, but not for those that do. Finally, suggestions are made for dealing with missing data.  相似文献

Lingual mandibular cortical defects (Stafne's defects) are relatively uncommon in recent as well as past populations, but while this condition is often discussed in clinical reports, they are rarely the subject of anthropological research. In this paper, the prevalence of Stafne's bone defects in skeletal samples from Late Neolithic, Late Roman, Medieval and Modern Portugal is investigated (N = 704 complete mandibles and 111 incomplete mandibles). The aims of this paper are threefold: (1) to present and analyse for the first time in Portuguese osteological record prevalence data on Stafne's bone defect, (2) to analyse variations in defect prevalence between skeletal samples from a wide temporal array and (3) to verify if more tenuous lesions in which resorption of the lingual cortex was not yet extensive had the classic radiographic appearance described by Stafne in order to validate the hypothesis that lesions are present clinically in many more cases than published figures indicate. In all cases, differential diagnosis against other conditions that mimic Stafne's defects, namely odontogenic lesions, cysts and neoplasms, was done. In all samples, the evidence of Stafne's defect occurred in 12 individuals (1.7%), and males (2.84%) were more frequently affected than females (0.61%). When incomplete mandibles were considered, only one left fragment (0.90%) exhibited Stafne's defect. Accurate identification of all examples of Stafne's bone defects in antiquity is thought to represent an important contribution to elucidate which factors may be responsible for this trait's cultural, ecological, temporal and geographical patterning. 我国传统木结构建筑的各构件间主要采用榫卯连接,这种节点形式在较大水平地震作用下容易出现局部拔榫、节点松脱现象,不利于结构的稳定和承载。为此针对榫卯节点的抗震性能及加固进行试验研究。依据清工部《工程做法则例》设计制作了4个足尺透榫节点试件,其中3个试件分别采用竹斜撑、角钢和碳纤维(简称CFRP)布进行加固。在改进榫卯节点拟静力试验装置的基础上完成了低周反复荷载试验,分析了对比节点和加固节点的破坏机理、承载力、延性、刚度退化、耗能和榫头拔出过程。试验结果表明,竹斜撑、角钢和CFRP布加固节点的正向峰值荷载较对比节点分别提高了92.4%、54.6%和63.0%,反向峰值荷载则分别提高了61.5%、75.7%和39.0%。对比节点和角钢加固节点初期刚度基本相同,且随转角增加出现先增大后减小的过程;竹斜撑加固和碳纤维布加固节点的初始刚度显著大于对比节点,随着转角的增加呈单调减少。此外,竹斜撑和角钢加固节点的榫头拔出长度明显小于相同转角下对比节点的榫头拔出长度,表明竹斜撑和角钢均起到了延缓榫头拔出的作用。  相似文献

为保护古建筑,采用试验方法,研究了马口铁加固古建筑榫卯节点后对节点抗震性能的影响.基于故宫太和殿三次间结构原型,制作了1:8缩尺比例的木结构空间框架模型,并考虑梁柱连接为燕尾榫形式,进行了低周反复加载试验,其中未加固构架和马口铁加固构架试验各做了3组.基于试验相关数据,获得了节点的弯矩-转角滞回曲线和骨架曲线,并对比分析了节点加固前后的耗能能力,刚度退化和延性等抗震指标.结果表明:马口铁加固榫卯节点后,虽然节点耗能能力略有下降,但节点拔榫量减小,抗弯承载力提高,刚度退化不明显,且节点仍具有较好的变形能力.因此,马口铁具有较好的加固效果.  相似文献

This article presents the results of an experimental study on the rotational behavior of degraded Chinese traditional mortise-tenon joints with different degradation types and different degradation degrees. Six joint specimens degraded through artificially simulated method, were tested by reversed cyclic loading, from which the moment-rotation curves were obtained. The results indicated that the rotational behavior of these joints is semi-rigid, the hysteretic curve shapes of degraded joints are same to that of joint without degradation. The maximum moment, yield moment, and initial rotational stiffness of degraded joints decreases as the degraded degree increases, whose empirical degradation relationships were obtained based on the test data. A hysteretic model for degraded traditional mortise tenon joints was proposed. Experimental results were used to validate the proposed hysteretic model. Good agreement between predictions and tests was observed.  相似文献

War and healthcare are inextricably linked. In fact, the prevalence of disease in the armed forces can inflict more damage than the enemy. The case of endemic malaria during the Italian campaign of 1943–1945 in the Second World War is an excellent example. The region of Puglia was one of the most malarial regions in Italy and provides a noteworthy case of Anglo-American endeavours to eliminate the mosquito during the Second World War. The high prevalence of endemic malaria in the area represented a substantial threat to the health of Allied forces and as a result to the Allied war effort. This article explores Allied efforts to protect both Italian civilians and Allied troops from malaria, in order to demonstrate how Allied health policies affected the Allied–Italian relationship.  相似文献

The results of some simulated seismic load tests on reinforced concrete one-way interior and exterior beam-column joints with substandard reinforcing details typical of buildings constructed in New Zealand before the 1970s are described. The tests were conducted using both deformed and plain round longitudinal reinforcement. The interior beam-column joint cores lacked transverse reinforcement and the longitudinal bars passing through the joint core were poorly anchored. The exterior beam-column joint units contained very little transverse reinforcement in the members and in the joint core. In one exterior beam-column joint unit the beam bar hooks were not bent into the joint core. That is, the hooks at the ends of the top bars were bent up and the hooks at the ends of the bottom bars were bent down. This anchorage detail was common in many older buildings constructed before the 1970s. In the other exterior beam-column joint unit the hooks at the ends of the bars were bent into the joint core as in current practice. The improvement in performance of the joint with beam bars anchored according to current practice is demonstrated. In addition, tests were conducted on interior joints with lap splices in the beam longitudinal reinforcement bars near the column face. The tests were conducted using both deformed and plain round longitudinal reinforcement. Tests were also conducted on columns with plain round bar longitudinal reinforcement and inadequate transverse reinforcement.

The reinforcing details were close to identical to those in an existing seven storey reinforced concrete building that was designed and built in New Zealand in the late 1950s.

The test results give an indication of the performance of beam-column joints and members with the above now out-of-date reinforcing details.

The test results reported are a summary of results reported in a number of publications written since 1994.  相似文献
