The U.S. National Health Interview Survey (NHIS) is the world's longest survey time series of health data and a rich source of information on health conditions, behaviors, and care from the 1960s to the present. NHIS public-use files are difficult to use for long-term analysis, due to complex file structure, changes in questionnaire content, and evolving variable names and coding schemes. Researchers at the Minnesota Population Center have created the Integrated Health Interview Series (IHIS) to overcome these problems. IHIS provides access to thousands of consistently coded and well-documented NHIS variables on the Internet and makes it easy to analyze health trends and differentials. IHIS multiplies the value of NHIS data by allowing researchers to make consistent comparisons over half a century and thus to study U.S. health status as a dynamic process. This article describes the main features of IHIS and suggests fruitful avenues for historical research using these invaluable health data.  相似文献

Women's health became intertwined with the second wave of feminism in the 1960s. The process and history of the interactions among physicians, feminists, health care consumers, activists, and politicians have brought women's health into the foreground of health research and curricular agendas in several ways. It identified major gaps in research and practice and initiated a critique of the current health care system at all levels; it originated from, and remains connected with, the non-medical community; it is interdisciplinary and requires interdisciplinary teams for research and clinical practice, as well as collaboration with colleagues in non-medical academic fields; it developed new curriculum focused on women's health; it emphasizes race, class, sexual orientation, and other diversities among women. The continuing presence of activists and feminist scholars will insure that health and science include women's needs to provide better health for all.  相似文献

Economic transition has been defined by neo-liberal restructuring policies and understandings. Using ethnographic data from Omsk, Russia, I examine structural adjustment policy implementation in the context of socially constructed gender norms. These policies have complicated implications for women and men's economic survival. The ethnographic understandings gained from interviews with women provide vital information that would improve planning processes in Omsk. For example, using an economic gardening approach to support women's small business development and workforce development targeting survivors of violence would advance women's economic self-sufficiency. I suggest that if planners use ethnographic understandings they will be able to more effectively respond to planning challenges such as poverty, education and health care issues.  相似文献

The newly named Commission on Population and Development, at its 1995 session in New York, stated it should have the primary responsibility for reviewing the implementation of the Program of Action of the International Conference on Population and Development, held in Cairo in September 1994. The Population Commission was renamed the Commission on Population and Development by the General Assembly. In debate, some countries reviewed their national experiences in population matters, stressing how the Program of Action would influence their policies. At mid-1994, world population stood at 5.63 billion persons, a report on world population trends and policies revealed. During the preceding 12 months, population had grown by 86 million and was expected to grow by an additional 87 million during the next 12 months. While China's representative described a near-crisis situation marked by a net increase in population of 14 million per year in her country, the representative of the Russian Federation expressed concern over the decrease in Russia's population. Between 1990 and 1994, world population had grown at 1.57% per annum. UN medium-fertility-variant projections indicated that the population growth rate would continue declining to 1.33% per annum between 2000 and 2010; 1.15% in 2010-2020; 0.95% in 2020-2030; 0.72% in 2030-2040; and 0.54% in 2040-2050. Consequently, the world population was projected to reach 7.5 billion by 2015 and 9.8 billion by 2050. Assistance for population programs had increased from $220 million in 1988 to $305 million in 1992, the bulk of which came from UNFPA. Family planning, integrated with reproductive health and carried out through a primary health care system, continued to receive the bulk of multilateral resources. Projects to improve the status of women, such as education for girls, leadership training, literacy programs, and projects designed to increase the income of women and their families were being supported by multilateral agencies. An update on global contraceptive requirements from 1994 to 2005 estimated that the total cost of contraceptive commodities would be $7.7 billion.  相似文献

ABSTRACT The authors have worked in Australian Aboriginal communities within the Wiradjuri area of central‐western New South Wales. Examining what appear to be distinctive Aboriginal approaches to time, we argue that these stem not from a different notion of time as such but, rather, from the relationship between the social and the self which places a distinctive value on the use and management of time. One way to access the dynamic between time and self is to realise that life is understood as fluid and contingent rather than predictable. This continually subverts the idea that time is measurable and controllable; that life is lived within domesticated sedentary space; and that planning ahead and self‐discipline are virtues. Yet these are notions central to practices associated with contemporary health care. A majority of health care providers, whether Aboriginal or not, are trained in the Australian mainstream health system and may consequently underestimate the implications of different ways in which a person acts on the temporal/spatial dimensions of her life, and how this influences ways in which she manages time in relation to her health and well‐being. Temporal concepts, such as ‘planning’, ‘discipline’, ‘future’, ‘boredom’, or ‘patience’, as well as that of the ‘long‐term’ with regard to managing illness or money, interact with the ways in which Aboriginal people experience themselves as ill or in need of health care, influencing how they act on medical advice. We argue that the key to understanding the use of time lies not in the concept of time per se but in what is involved in developing a responsive social self when the time/space dimensions of the day to day are informed by a fluid and thus contingent ontology of that day to day.  相似文献

The success of persons with severe physical disabilities in living independently in the community, engaging in self-directed productive activities, and staying healthy depends, in part, on getting and maintaining adequate personal assistance with basic tasks such as getting out of bed and tending to personal hygiene. Although few policymakers would disagree with the contention that many people with disabilities need assistance with daily tasks, there is considerable debate about who should provide assistance, what services should be included, who should pay, who should be entitled to publicly provided and paid services, how much assistance should be provided, and who should control its administration. The prevailing model for delivery of personal assistance services has been primarily medical, with control of services by health care professionals, rather than independent living, with control by consumers receiving services. Traditionally there has been an institutional bias of funding for disability-related service, despite its much higher expense than community-based personal assistance services and its violation of basic human rights. Proponents of various solutions to health care reform have given little consideration to the provision of support services for people with disabilities. The advantages of implementing a uniform national system for the delivery of personal assistance services based on consumer control are discussed.  相似文献

The ideal of a comprehensive higher education planning agency was only partially fulfilled in the 1970's. It was especially limited in its ability to bring independent colleges and universities into statewide planning. The unfulfilled comprehensive statewide planning model makes an attractive case for decentralised decision–making. State planning, usually characterized by the so–called “bureaucratic model,” might be balanced by increased use of an “incentive model.” The work of the state agency would be to encourage and reward fair market competition among the institutions and segments of higher education. Such an approach is based on the conviction that “…intelligent planning can diminish the role of government in the academic and administrative affairs of all sectors, while improving accountability to the public and to the diverse constituencies our institutions serve.”  相似文献

Since the mid-1980s, U.S. tobacco policy has been an intense and acrimonious issue between antitobacco advocates and the tobacco industry. In the United States, the tobacco industry has responded to heightened state antitobacco litigation, adverse public opinion, and public health advocacy by aggressively mobilizing against tobacco taxes and regulations. This article examines whether these tobacco policy trends can be generalized to punctuated equilibrium theory ideas that policy monopolies are stable over long periods and usually change because of sharp and short-term exogenous shocks to the policy system. From 1990 to 2003, there was a sharp mobilization by health advocates in all states and a significant rise in new legislation to control tobacco use. The tobacco industry, nevertheless, was able to generally keep state tobacco taxes low and counter significant regulatory threats to tobacco sales. From this, I conclude that the policy monopoly favoring the tobacco industry did not significantly change, despite the symbolic appearance of punctuation in the policy system.  相似文献

In 1921, Pearl S. Buck gave birth to a daughter, Carol, who became severely retarded and was eventually institutionalized at the Vineland Training School in New Jersey. To help pay for her daughter's care, Buck wrote The Good Earth in 1931, and then other novels and biographies about her life in China, for which she was awarded the Nobel and Pulitzer Prizes, and honored around the world. Years later, she published The Child Who Never Grew, a short piece about her daughter's retardation that also revealed her desperate search for answers and good clinical care. Asbj?rn F?lling distinguished phenylketonuria (PKU) from other forms of childhood retardation in the mid-1930s, and new assays and biochemical findings eventually led to ways to circumvent the devastating effects of PKU. But for Carol Buck, these advances came too late. It was not until the 1960s that physicians confirmed that her severe retardation was caused by PKU.  相似文献

Social scientists have studied the welfare state extensively. Many studies seek to understand the determinants of the welfare state; however, a few have explored the social consequences of social welfare systems, especially on health outcomes of the population. Even though cross-national comparative studies support the thesis that the welfare state regime type, which represents different levels of commitment on social welfare, is closely linked to population health, there is little research to support this argument at a sub-national level. To fill the gap, this study explores the effects of the U.S. states' social welfare systems on health using age-adjusted mortality rates as a proxy for population health. By operationalizing social welfare systems as three dimensions—public expenditures, tax structures, and welfare program rules—we find that more generous education spending, progressive tax systems, and more lenient welfare program rules help to improve population health. The model corrects for first-order serial correlation using Prais-Winsten regression methods and is estimated with state and year-fixed effects.  相似文献

Abstract

The passage of the Affordable Care Act (ACA) in 2010 marked a significant achievement for congressional health reform. Despite increased partisanship, decreased productivity, and increased polarization of Congress, major health reform legislation was passed. We compare the ACA’s passage to the failure of Clinton-era health reform, emphasizing gridlock theory, which some scholars interpreted to predict that major health reform would not be possible in 2010. We identify several ways in which the different outcomes for the ACA and the Clinton reform effort can be explained by gridlock theory: pivotal actors, the gridlock window, and the status quo. However, factors not included in gridlock theory, such as the role of parties and institutional change, appear to also have contributed to the success of the ACA relative to the failure of Clinton health reform. Lessons learned from health reform suggest an opportunity to expand gridlock theory to account for these additional factors.  相似文献

Planning systems are generally divided into plan-led and development-led systems. However, it is quite common that the planning practice follows development-led planning although the planning system would structurally be plan-led. To study how development-led planning affects the implementation of large-scale urban development projects, we conducted a cross-sectional survey in Finland – a country with a hierarchical setting of legally binding zoning plans which can be prepared by following either plan-led or development-led practices. The survey respondents were civil servants managing implementation of large-scale urban development projects. Our results quite surprisingly suggest that development-led planning may cause the projects to become more vulnerable to property market uncertainty compared to the projects following the plan-led practises. These results challenge the usual output-based legitimation of development-led planning to some extent.  相似文献

The interim Egyptian government's excoriation of U.S. support for nongovernmental organizations (NGOs) in the country has sparked a crisis that some analysts have called the worst deterioration of United States–Egypt relations in history. As Cairo's smear campaigns against the civil society community foment public mistrust among many Egyptians of NGO activity and foreign funding, U.S. policymakers and practitioners face new challenges in supporting civil society work in Egypt. For a number of reasons, however, Washington's assistance to Egypt should and almost certainly will continue, even if the environment for civil society activity in the country does not improve. Grantors and implementers must think seriously, therefore, about long‐term strategies for assisting civil society development in Egypt, which will require at least some coordination with a government that may be suspicious of U.S. efforts. By standing firm on red lines, improving public messaging in Egypt, carefully fostering local ownership of projects, remaining strictly neutral in identifying grantees and diversifying partnerships, distinguishing between short‐term foreign policy objectives and long‐term efforts to assist civil society development, and using varied democracy assistance tools appropriately, the United States can assist NGOs in Egypt in a way that gives them—and democracy—the best chance for success.  相似文献

集体化时期的合作医疗制度始终坚持缓解农村医疗困境的大方向,通过发挥集体经济的优势、进行医务人员"在地化"培训以及坚持"预防为主"等方式,让有史以来人数最多的农民有了制度化的健康保障,充分体现了社会主义制度的优越性,并赢得国际社会的广泛赞誉。取得如此成就的原因,国家领导人的高度重视、国家的政策倾向,以及"文化大革命"的推动缺一不可。而这些因素发挥作用,又必须以集体经济为依托。虽然合作医疗制度在具体操作中也存在基层干部及赤脚医生玩忽职守、营私舞弊等问题,但其在国家一穷二白的条件下所取得的成就依然是共和国医疗卫生史上的制高点。  相似文献

This article reports on the work carried out by the UN Population Fund (UNFPA) in the small island of Djibouti, Africa. The republic's population has been plagued with problems of high levels of unemployment, poverty, malnutrition, an almost non-existent family reproductive health care service, 100% prevalence rate of female genital mutilation and low literacy rate, especially for women. In addition, refugees from Ethiopia and Eritrea have settled in the country increasing the risks of sexually transmitted diseases (STDs), HIV/AIDS, prostitution, and other social ills. In 1983, UNFPA started funding family planning and later reproductive health projects aimed at assuring access to services for a majority of Djiboutan women. The first country population program of assistance was started in 1992. This would help the government with health care for its population and to conduct a population census. In addition, the Fund has paid for training of doctors, midwives, and traditional birth assistants in the country and for rehabilitating maternity clinics and information centers. Moreover, it has supported agencies concerned with educating people on STDs, HIV/AIDS, safe motherhood and reproductive health for men and women, and other important issues.  相似文献

Our neoliberal governance model places a burden on planning to often take “responsibility” for the failure of market-lead governance to deliver its policy promises of betterment, security and future enjoyment. These include promised, but often-unachievable policies, such as those of increased global competitiveness for areas of structural economic decline; or housing affordability in areas of population growth and constrained land availability. Resultant policy failures then result in a scapegoating response where planning is held responsible. Examples include that economic development, or housing affordability, is obstructed by planning impediments, such as regulatory controls or process delays, which are claimed to hamper efficient market delivery. To deconstruct this neoliberal fantasy that planning often impedes policies for market-lead success, the article will first document exemplars of this scapegoating process. It will then explore the role of fantasy and ideology in governance policy formulation and, from a Lacanian perspective, the theorization that underlies this process. Then, it will investigate the role of the “scapegoat” for public policy facilitation so as to explain why planning is often placed in this role, and why this role is often ideologically necessary, at least for neoliberal governance, when planning undertaking its statutory responsibility of facilitating the public interest.  相似文献

The creation of the National Assembly for Wales in May 1999 was part of a series of constitutional reforms in the UK initiated by the Labour Government after 1997. The Assembly, that has been awarded policy-making powers only, is still in its infancy, and any assessment of the perceived success of the new forum would be premature. Nevertheless, there are signs emerging that could give rise to the view that the Assembly will carve out a degree of distinctiveness in its approach to governance and substantive policy areas compared to that operating in England generally, and to the land use planning system in particular. This review paper charts the birth of the National Assembly for Wales, outlines its responsibilities and functions in relation to planning, and assesses what has happened 'on the ground' in its first 18 months. Overall, the paper portrays politicians and officials in the new governance framework searching for distinctiveness, inclusiveness and policy ownership; the task is to offer something different compared both to the system existing previously and to the on-going processes of institutional restructuring underway in other parts of the UK. It situates planning within the crucible of decentralization and devolution, since planning has been awarded a central facilitating role within the governance and restructuring process as part of the Welsh Cabinet's determination to deliver sustainable development in the twenty-first century.  相似文献

The publication in 1987 of a Soviet compendium of population statistics has made possible the calculation of net migration balances for the U.S.S.R.'s 20 large economic planning regions by the residual method for the first time since the middle of the 1970s. This paper compares the results for 1981–1985 and 1971–1975 and finds that Siberia has changed from a net loser to a net gainer of migrants, that the shift of population to the southern regions has been reversed, and that the traditional rural outflow from central-eastern European Russia has diminished. Nevertheless, the acclimatization of newcomers in the eastern regions, the stabilization of rural dwellers in the central regions, and the mobility of rural residents in Central Asia and the Transcaucasus all remain insufficient to overcome regional imbalances in labour.
La publication en 1987 d'un compendium Soviétique sur les statistiques de la population a rendu possible le calcul des balances migratoires pour les vingt grandes régions économiques de l'U.R.S.S. pour la premiere fois depuis le milieu des années 1970. Cet article compare les résultats de 1971–1975 et 1981–1985 et montre que la situation en Sibérie est passée d'une perte nette à un gain net de migrants, que le mouvement de la population vers les regions du Sud a été renversé, et que le traditionel exode rural observé dans les régions centrales et dans l'est de la Russie europeenne a diminue. Néanmoins l'adaptation des nouveaux arrivants dans les regions de l'est, la stabilite des ruraux dans les regions centrales, et leur mobilite en Asie Centrale et en Transcaucasie sont insuffisants pour surmonter les desequilibres regionaux en main-d'oeuvre.  相似文献
