In late life, the humeral impingement disorder becomes common due to degenerative changes in the rotator cuff which allow the proximal humerus to press against the acromion in elevation of the arm, so producing facets on the tip region of the subacromial surface and on the lateral margin of the greater tubercle of the humerus. To discover what changes occur prior to those described, 184 acromions below the age of 55 years were examined. During the 18–25 year period, the subacromial surface showed patterns of formation of surface trabeculae of new bone, some associated with union of the epiphysis or with remodelling as a response to the presence of the proximal humerus at a distance of 10–15 mm; others indicated a subacromial attachment of the coraco‐acromial ligament. As early as 30 years of age, flattening of the surface trabeculae of bone was noted in association with compression lipping of the outer margin of the greater tubercle of the humerus, the part that crosses closest to the subacromial surface during elevation of the arm. Although resembling changes found in the impingement disorder, in both populations by the age of 35 years the changes were too common to be so and were not associated with the erosive pitting of the upper surface of the tubercle regarded as the earliest bony criterion of degenerative change in the cuff. It is postulated that, in the normal shoulder joint, in raising the arm, pressure is exerted, perhaps transiently, in the subacromial bursa and adjacent soft tissues which induces the compressive changes described. Probably due to the physically arduous lifestyle of the islanders, these changes occurred earlier among them; so did (but later) cuff degeneration indicated by erosion of the humeral tubercle. Apparently-articular facets on the undersurface of the anterior third of the acromion have been known in skeletal material since 1922 but without full discussion of the mechanism responsible. Twelve instances are described here, selected because an impingement area could be demonstrated on the greater tubercle of the humerus or on osteophytes that had replaced it. Both sides were affected but, with two exceptions, the right more severely; the less affected side could be regarded as an earlier stage. All were affected by osteoarthrosis. One was complicated by coraco-humeral impingement. The anatomical mechanism that normally prevents the humerus from rising against the coraco-acromial arch is described as well as the causes of the breakdown in the mechanism that leads to the impingement, derived largely from the clinical field, where the disorder is well-known. The complexity of the relation of the disorder to degenerative arthrosis is discussed because the degeneration is an age change in which wear and tear plays a localized part and the changes as they affect the shoulder joint differ fundamentally from those in the weight-bearing hip-joint. The mean estimated age of the group of twelve was 59 years but the occurrence of the acromion impingement disorder in young athletes illustrates the part played by mechanical joint abuse. Women predominated in this small group, partly because, through their longer life-spans, their joints were exposed longer to the susceptible period of age-related degeneration and also because their less robust musculo-skeletal systems were less adapted than men's to the equally shared labour. The humeral impingement disorder (HID) results from degenerative changes in the rotator cuff tendon which allows the humerus to press against the acromion and produce pressure facets on both the acromion and the greater humeral tubercle. A previous study of those below 55 years of age in the two populations defined a system of three morphological categories of subacromial features. The first two categories were postulated to be pre‐impingement while the third, distinguished by erosive pitting at the point of attachment of the cuff to the greater tubercle, was regarded as indicative of degenerative change in the cuff and, therefore, of HID. Extending the study to those over 55 years of age has now added two categories of more advanced HID change. Those on the right side, of dominant use, were always more severe, thus enabling a less severe Category 4 to be distinguished from Category 5, the chief feature of which was a raised facet which could be large or small. Confirmation of the earlier study's conclusions was achieved: the degenerative changes of Category 3 began to be common by 40 years of age in both sexes of the Scottish island people, living physically‐stressed lives; in the more wealthy and sedentary Londoners, this does not occur until 10 years later. Nevertheless, in both sexes in both populations, virtually all those over 65 years of age showed Category 4 or 5 change. It seemed likely that a higher proportion of the Londoners reached advanced ages than among the islanders and so had more time for the progressive disorder to show its full effects; for instance, there were 16 Londoners known to be over 80 years of age. Individuals of that age cannot be distinguished by methods for estimating age from those 15 years or so younger but it was regarded as unlikely that there were more than two or three octogenarians among the islanders. Two complete skeletons from the Hamman-Todd collection of the Cleveland Museum of Natural History (CMNH) show a suite of pathological lesions that suggest the individuals had been involved in boxing or other hand-to-hand combat. These lesions were studied and compared with medical and autopsy records. The aims of the research were to estimate the accumulated damage to the bones over time, to characterize the different types of the damage, and to establish criteria for hand-to-hand combat or violence for archaeological material. Our inspections showed that besides the muscle markings developed and the numerous healed fractures that are expected when someone is involved in such activities, other types of lesion are present that are helpful for a proper differential diagnosis. These are: degenerative changes at the lesser tuberosity of the humerus; focal necrotic changes/bone growth on the trochlea of the humerus; necrotic changes on the distal head of the ulna; bony patches on upper limb bones only; secondary centres of ossification failing to fuse (mainly in vertebrae and acromion); a huge conoid tubercle on the clavicle; bony spurs on the distal articular head of the metacarpals; necrotic changes on the femoral head next to the fovea and on the roof of the acetabulum; and a developed bony ridge for the attachment of the iliotrochanteric ligament. Finally, we propose a set of criteria that will help to identify people in archaeological material who were involved in hand-to-hand combat.  相似文献

This paper details the differential diagnosis of an adult female skeleton displaying features consistent with a cartilaginous dysplasia and Osgood–Schlatter's disease. This burial was excavated in 1940 from a Mississippian platform mound at the DeArmond site (40RE12) in Roane County, Tennessee as part of the Works Progress Administration (WPA) and Tennessee Valley Authority (TVA) archaeological investigations. The right humerus and left femur of this individual display traits consistent with achondroplasia, such as shortened length and normal width. However, the rest of the long bones display typical morphology. The affected humerus and femur are 82 mm and 58 mm shorter than their counterparts, respectively. This makes for obvious asymmetry. A review of the various cartilaginous dysplasias was undertaken to identify those conditions concordant with the differential manifestation of the long bone asymmetry. Morphological and radiographic analyses were used to rule out possible diagnoses. The most likely candidate is enchondromatosis out of the presented conditions, as it most commonly affects the long bones, does not affect every bone in the skeleton, and is asymmetrical in its manifestation. In addition to the cartilaginous dysplasia, both anterior proximal tibial metaphyses of this individual display defects that are roughly triangular with pitted, irregular floors. Osgood–Schlatter's disease is caused by repeated trauma to the tibial tuberosity during childhood. This individual would have walked with an obvious limp, and perhaps the added biomechanical stress on both quadriceps muscles as a result caused the injuries and resultant defects to its attachment points. Evidence of these conditions in an adult indicates that the Mississippian people in this community offered some sort of social support to differently‐abled individuals. The main goal of this paper is to describe and discuss pathological lesions observed in a Roman skeleton (between 2nd and 3rd century AD) from the north‐east region of the Iberian Peninsula (St Nicasi 18–24 site. Gavà, Barcelona), which may be compatible with treponematosis. Most of the skeleton, with the exception of the neurocranium, was recovered. Only the left tibia was affected, whereas the rest of the recovered skeletal remains were unaffected. Macroscopic examination revealed a male individual between 25 and 30 years of age at death with a sabre‐shaped left tibia. The proximal half of the diaphysis was pitted and the bone overall enlarged. The surface of the tibia showed occasional vascular impressions where, in some instances, small raised plaques of new bone appeared to bridge over them, specifically in the most affected area of the proximal half of the tibia. No destructive lesions were observed. Radiographic examination and gross inspection at the cross section of the tibia showed encroachment into the medullary cavity of coarse cancellous bone and cancellization of the cortex. The observed lesions indicate that the tibia was affected by a chronic infectious disease. Differential diagnoses were considered, and these included other infectious diseases, fibrous dysplasia, Paget's disease, chronic varicose ulcers affecting bone and trauma, with the conclusion that the disease affecting the tibia could have been treponematosis. Internationalization has been an important concept in higher education over the past two decades. The way that internationalization is manifest has consequences to academic disciplines, including geography. A new system of liberal arts colleges in the Netherlands has created opportunities for interdisciplinary education, representing a departure from the traditional model of European higher education. At Leiden University College the focus on global challenges created an opportunity for geography to be integrated into a diverse interdisciplinary and international curriculum. The positioning of geography into the curriculum, however, occurred because its adaptability and breadth, which enabled it to be incorporated into diverse facets of the overall teaching programme. To be successful, however, geographers should be cognizant of specific facets of the discipline suited for an international context. The case study herein reviewed highlights important differences between the U.S. and Dutch (European) context. Additionally, important points for academic geographers (academics) who are in interdisciplinary programmes (i.e. not a geography department) are emphasized, which relates to the development of course materials to suit an international context. The identification of key thematic and methodological disciplinary strands serves as bridges to interdisciplinary opportunities, which was essential to geography’s successful establishment within an international curriculum.  相似文献

Comparison of the ultrastructure of the hyaline tissue of conodont elements and the enamel of vertebrates provides little support for a close phylogenetic relationship between conodonts and vertebrates. Transmission and scanning electron microscopy shows that the mineralised component of the hyaline tissue of Panderodus and of Cordylodus elements consists of large, flat, oblong crystals, arranged in layers that run parallel to the long axis of the conodont. Enamel in the dentition of a living vertebrate, the lungfish Neoceratodus forsteri, has crystals of calcium hydroxyapatite, arranged in layers, and extending in groups from the dentine-enamel junction; the crystals are slender, elongate spicules perpendicular to the surface of the tooth plate. Similar crystal arrangements to those of lungfish are found in other vertebrates, but none resembles the organisation of the hyaline tissue of conodont elements. The crystals of hydroxyapatite in conodont hyaline tissue are exceptionally large, perpendicular or parallel to the surface of the element, with no trace of prisms, unlike the protoprismatic radial crystallite enamel of fish teeth and scales, or the highly organised prismatic enamel of mammals.  相似文献

Terqa, situated on the right bank of the Middle Euphrates, is known to have been a site already in the third and second millennium BC. Excavations which take place in this region aim to provide answers for numerous significant issues connected with the origins of human civilisation. In 2008 season we found a tomb dated 2650–2450 BC, consisting of two chambers with stone domes. The smaller chamber contained many luxury grave goods. The other one was bigger and contained human skeletons. The first skeleton belonged to a man, 45/50 years old. It is extremely heavy and large. On the right humerus, near the proximal edge, we found two cuts. The healed edges of the wound suggest that the man from Terqa survived after the wound was inflicted. Many muscular attachments were clearly marked on the bones and bone robustness was far above the average, which may suggest that the skeleton belonged to a warrior. These observations correspond to the fact that the bronze part of a belt together with bronze weapon‐blades was found on the right side of the hip. The second skeleton, which belonged to a female who was about 40/44 years old, was found in an anatomical position. The chamber also contained an almost complete skeleton of a sheep. The morphology of the forearm of the female suggested strenuous activity. From this skeleton was successfully isolated HVR1 fragment. The habitual adoption of certain gestures and postural habits can result in a variety of non‐metric traits, including supernumerary facets that are both articular and non‐articular. The squatting facet is a classical example of the former, and generally presents as a lateral articular facet on the neck of the talus that articulates with a facet on the anterior border of the distal tibia. These facets arise owing to the adoption of the squatting position and the hyperdorsiflexed position of the ankle joint, bringing the talus and tibia into immediate approximation. This study reports on the difference between the classical squatting facet and a non‐articular facet, which can also present on the neck of the talus but occurs, probably, as a result of soft tissue pressure, rather than bony articulation. The human right clavicle tends to be shorter than the left. A range of possible explanations can be advanced to account for this asymmetry. In the present work, clavicular morphology was studied in a medieval skeletal assemblage from Wharram Percy, England, with the aim of evaluating these competing explanations. At Wharram Percy, as anticipated, the right clavicle tended to be shorter. Patterns of asymmetry in curvature, robusticity, vascularization, and in the morphology of ligament and muscle attachment sites were also studied. The patterning in these data at Wharram Percy is consistent with inhibition of longitudinal growth of the clavicle on the dominant side due to lateral bias in mechanical loading, particularly in axial compression. Located on the southernmost tip of Israel, at the meeting point of the Negev desert and the Red Sea, Eilat was occupied in March 1949 in the last military operation of Israel's War of Independence. Notwithstanding official notions of Eilat as a strategic asset and efforts to sustain Israeli presence there in the form of a permanent settlement, in Israeli popular culture Eilat, which later evolved into a thriving tourist resort, has had from the outset the reputation of being ‘different, special, distinct’. This article explores the unique position of Eilat in Israeli popular culture. The underlying argument is that this unique position of Eilat both evinced and reflected a dynamic interplay of liminal conditions and phenomena that rendered Eilat an exceptional experience. Furthermore, being situated at the southernmost tip of Israel, the location of Eilat at the geographical margin of Israel was productive of its image as an extraordinary place and sustained its prominence in popular imagination. The article charts and analyses a variety of liminal phenomena in their social and cultural contexts in different periods of Eilat's history.  相似文献

This paper reports results of a recent Stone Age-focused archaeological survey in the Red Sea coastal region of the Republic of Sudan, northeast Africa. Bifaces (handaxes) are the most conspicuous artifact class encountered during the survey and are characteristic of the Acheulean technocomplex. Other recorded artifact types include points, scrapers, and prepared core products referable to the Nubian and recurrent Levallois methods. Most of the artifact-bearing localities lie landward—outside of the coastal margin—thus, the evidence does not signify direct coastal adaptation per se. Our preliminary findings suggest that multiple Pleistocene-age hominin settlements tied to a terrestrial niche existed in the region. The western margin of the Red Sea occupies a pivotal location, linking the Horn of Africa and the Levant, two vital regions in human evolutionary research. Thus, the Stone Age data from the Sudan region has direct relevance for assessing hominin dispersal routes out of Africa.  相似文献

A male human skeleton from Thrace dated at around 320 BC was found in a wealthy tomb 12 km from what is now Tekirdağ, Turkey. The occupant of the tomb died at the age of 40–45 years and has a pathologic left humerus caused most probably by traumatic injury. It is 9 cm shorter than the right humerus. A drainage fistula penetrating into the medullary cavity occurs at its proximal end. Due to severe arthritic destruction, the left humeral head has completely lost normal articulation, with the glenoid cavity of the left scapula displaying, in turn, severe erosive lesions and important reduction in its articular surface. A marked abscess is discernible bilaterally on the chondro‐costal sternal end. The individual also has a hip with a total sacroiliac joint fusion and shows slight or moderately developed exostoses on different parts of his skeleton. These joint destructions most likely indicate that he suffered from chronic osteomyelitis and septic arthritis. Copyright © 2005 John Wiley & Sons, Ltd.  相似文献

Gunshot trauma is an important, but often overlooked, avenue for interpreting the historic past. Ten skeletons of Seventh Cavalry troopers, killed during the Battle of the Little Bighorn in 1876, are analysed osteologically. Six of the ten specimens have gunshot wounds, including three to the cranial vault. The other wounds are to postcranial remains, including one in an innominate. This wound location aids the identification of the individual, confirming and complementing historic accounts. Of the remaining two, one is to the humerus and the other to the chest region. These wounds provide insights into the nature of perimortem injuries, weapons used in the battle, and interpretations of the battle.  相似文献

Occipital bone lesions on an Iron Age horse cranium from the burial mound of Arzhan 1, Tuva, Central Asia, are described and interpreted. Cavitations around the nuchal ligament attachment site on the skull are interpreted as foci of inflammation and necrosis following local infection. It is suggested that the pathology represents a case of ‘poll‐evil’, most likely due to a bacterial infection. The significance of such an interpretation is discussed, including its implications for disease ecology and the possible infection risks to contiguous animal and human communities of the first millennium BC in Central Asia.
